How do I make a complaint about a long term care facility
Long term care facilities are also covered by the Department of Heath Services. You should also report abuse or neglect of vulnerable adults to Adult Protective Services. You can report abuse to APS at (877) 767-2385. You can also contact your local Long Term Care Ombudsman to see if they can assist you in resolving the problem with the agency directly. If you are not satisfied with your regional long term care ombudsman, or are not sure which region to contact, you can contact the DES oversight office (602) 542-6446.
How do I contact a Long Term Care Ombudsman
The best way to reach a long term care ombudsman is to contact your local Area agency on aging. See below for links to the different regions. Also included is the best number to reach the long term care ombudsman for that region.
In addition to the regional ombudsman, every state has a long term care ombudsman program administrator. In Arizona, that administrator is in the Department of Economic Security (DES). The administrator oversees the regional programs around the state. This office can be reached at 602)542-6446

What can I do if I believe an adult is being physically abused or exploited?
If you believe the adult is in life threatening danger, call 9-1-1. Otherwise, people can report abuse, neglect, and exploitation of Arizona’s vulnerable or incapacitated adults, 24 hours a day, seven days a week at the state’s hotline, 1-877-767-2385. If I have been denied medical assistance and cannot afford the cost of my prescriptions, are there any alternative options?
The CoppeRx Card gives Medicare-eligible residents a way to get substantial discounts on prescription drug prices. Generally, the card discounts range from 15 to 55 percent. The card is free of charge and is available to anyone 65 or older or disabled. You can call 1-888-227-8315 for more information.
How can I find resources that will enable me to take care of my elderly parents?
Contact your local area agency on aging. You will talk with a trained professional who can help you find convenient transportation, health care options, homemaking or any other service that can help you and your parent .The service is free.
